MyMedTell - Telemedicine Informed Consent Policy
MedTell is a telemedicine platform that facilitates patients by allowing them to get prescriptions from doctors and physicians online. As a telehealth platform, we’re not a replacement for in-person doctor visits. Instead, we’re a supplement meant to be used as a secondary means for getting medical attention to treat ailments.
MyMedTell Informed Consent
- I consent and authorize MedTell’s medical providers to assess my medical situation based on the information I’ve provided to provide me with the best course of treatment necessary to cure my ailment. I also release MyMedTell.com of any legal responsibility incurred by the medical practitioners on MedTell’s platform.
- I understand and acknowledge that MyMedTell.com is a telehealth platform. It’s not a replacement for in-person doctor evaluations and consultations. I also acknowledge that electronic communication will be used during my telehealth consultation with MedTell and that such consultations won’t be similar to an in-person patient-healthcare provider since I won’t be in the same location as my healthcare provider.
- I understand the risks associated with using electronic communication technology for telehealth purposes. These might include technical difficulties, internet disruptions, data breaches because of unauthorized access, etc.
- I acknowledge that MyMedTell.com may share my personal, medical, and healthcare data with its medical providers and my desired pharmacy staff to provide appropriate treatment plans and medication. MedTell’s medical providers will maintain the confidentiality of the obtained information.
- I understand that telemedicine or telehealth has several risks, including misdiagnosis, delayed diagnosis, or treatment failure, because medical providers operate on limited information. They’re not evaluating my condition face-to-face or using any diagnostic tools. Instead, they’re only using the medical and health information I’ve provided to form a prognosis. As a result, I understand the possibility of not receiving the proper or wrong treatment. I acknowledge these risks and am still going to proceed with an online consultation with MyMedTell.com.
- I understand the importance of seeking immediate medical care (usually within 48 hours) if symptoms worsen or don’t improve timely because of the potential risk of misdiagnosis or delayed diagnosis.
- I acknowledge that MedTell’s medical team operates on limited knowledge. Therefore, their treatment plan, therapeutic advice, and medical recommendations are based on rudimentary information. I agree to follow up with in-person doctor consultation for a re-evaluation if required.
- I acknowledge that electronic communication technology might not be sufficient for some medical conditions and health situations. I agree that the healthcare provider or I can discontinue the telemedicine visit if such situations arise.
- I acknowledge that I’m afforded the right to withdraw my consent for telehealth or telemedicine use at any time during the process. However, MyMedTell.com and its medical team may continue providing me with healthcare services through electronic communication technology as long as my consent is in force without signing additional consent forms.
- I acknowledge that I understand the alternatives to telemedicine and telehealth consultations. I choose to engage in an online consultation with this knowledge. I also understand that some medical evaluation procedures may not be possible or limited because I won’t be in the same room as my healthcare provider.
- I’m aware that MyMedTell.com uses synchronized and asynchronous communication to deliver healthcare. I have also visited MyMedTell.com to understand their scope of practice, telemedicine practices, procedures, etc.
- I certify that I must be a legal adult to use MyMedTell.com or have a legal guardian of adult age if I’m a minor to use the platform.
- I acknowledge that MEDTELL’S payment for services rendered are on a non-refundable basis
- I have carefully read MedTell’s payment policy and understand that the consultation fee doesn’t include the prescribed medication’s cost. Instead, it covers the physician’s fee. I acknowledge that I’ll need to pay for my prescribed medication at my selected pharmacy.
- I acknowledge that I will provide accurate and up-to-date information on the patient intake form to reduce the risk of misdiagnosis, wrong diagnosis, and treatment failure.
- I understand that failing to provide truthful information on the intake form could have severe consequences, such as the risk of receiving the wrong treatment or misdiagnosis.
- I understand and acknowledge MedTell’s right to refuse to provide treatment if a patient provides misleading information.
